Dr. Sam (Sarmad Eiwaz, DMD) has a true passion for dentistry and his patients. He takes the time to thoroughly listen to his patients’ oral health goals and needs, and then develops a customized treatment plan for them. He enjoys educating patients about their oral health and helping guide them to achieving their oral health goals.
Dr. Sam is a proud Chaldean American, having been born in Iraq before his family fled to the U.S. in the mid-90s. His parents settled in Farmington Hills. Dr. Sam briefly left Michigan while attending college in Florida, however, Michigan had stolen his heart and he returned following his graduation from dental school. He presently resides in Birmingham, MI, but can often be found in Farmington Hills where he frequently visits his parents, who still live in his childhood home – just two short miles from the office.
Dentistry allows Dr. Sam to beautifully blend his knowledge and skills in oral healthcare with his love of design and art. He’s able to masterfully envision each patient’s desired outcome when developing treatment plans. Dr. Sam takes great joy in seeing his patients’ dreams become reality.
Dr. Azimi completed his undergraduate studies at Virginia Commonwealth University in 2014 where he majored in Chemistry. He obtained his Doctor of Dental Surgery from the University of Detroit Mercy School of Dentistry where he served as Class President from 2014-2018. Soon thereafter he completed his Master of Science (MS) and Certificate of Advanced Graduate Studies in Periodontics and ;Dental Implants; at the University of Detroit Mercy School of Dentistry and St. John Hospital Health System in Michigan from 2018-2021. He is a Board-Certified Periodontist from the American Board of Periodontology and the State of Michigan.
After his periodontics residency, he began his active duty to serve in the United States Air Force. He completed his Officer Training School and Squadron Officer School at Maxwell Air Force Base to further advance his leadership and military training. He served at Joint Base Andrews from 2021-2024 as a Periodontist and Associate Professor in AEGD residency program.
Dr. Azimi is well-cultivated and experienced in all aspects of Periodontics and dental implant placement. His experience working in collaboration with Prosthodontists and Oral Surgeons in the Air Force using the latest technology allows him to apply treatment approaches from both restorative and surgical perspectives.
He enjoys teaching and takes pride in sharing his expertise in Periodontal treatment and dental implants with fellow clinicians and his residents. He is a member of the American Board of Periodontology (ABP), American Academy of Periodontology (AAP), the American Dental Association (ADA), and the Michigan Dental Association (MDA).
Dr. Azimi and his wife are very excited to be back in Michigan. He is currently working full-time in private practice and part-time with the Air National Guard. He is dedicated to serve his patients to address their periodontal and dental implant needs with commitment to excellence and patient satisfaction. He is fluent in Farsi and English. Him and his wife currently reside in Lake Orion. He treasures his time spent with family and friends, playing soccer, and travelling.
